Insurance Information
Your dental insurance is designed to assist you, the policy holder, with dental fees. However, few policies provide complete coverage like those found in the medical field. Normally, payment of our fees is your responsibility and is due at time of service.
Our insurance coordinator deals with many different insurance companies and is knowledgable about the coverage of most insurance plans. Sometimes these plans change faster than we (or you) can keep up with. Therefore, we will do our best to provide you with as accurate coverage estimates based on information available to us.
As a courtesy to you, with the exception of the initial examination appointment, we will file your insurance for dental services and accept assignment of insurance benefits. This coutesy is extended to you with the following provisos;
1. We must be able to verify benefits prior to the procedure.
2. Patient agrees to pay any portion of the fee that the insurance company does not cover for treatment. This would include deductibles, excluded services and restriction of benefits.
3. Patient agrees to monitor claims filed with the insurance company, by calling and checking the status of claims until claim has been paid.
4. Assignment of benefits is accepted for a period of 90 days from the date our office submits the initial claim to your carrier. Should your insurance company fail to provide benefits within this period of time, your remaining balance will become due and payable.
5. Major surgical services may be covered at less than expected Usual and Customary Rates ( UCR).
